Please supply the following information:
1. How many people have an advanced care plan (ACP) in place for the latest year that you have records for?
2. How may people, of that same year, have a terminal illness?

Thank you for your enquiry into advance care plans and terminal illness.
Unfortunately, we do not hold data patients with advance care plans or who have terminal illness. Our data is derived from information collected at the point of death registration.
