Please supply the following information:

1. How many people have died from COVID with the vaccination (2 doses)?

• Can you please give me the full total and then broken down by vaccination brand please?

• Can I also have this broken down by age group please?

2. How many people have died within 28 days of a positive COVID test with the vaccination (2 doses)?

• Can you please give me the full total and then broken down by vaccination brand please?

• Can I also have this broken down by age group please?

3. Is there a financial incentive for hospitals / GPs / NHS trusts etc for each person who has been vaccinated?

4. If so, is this for each jab or after the 2nd jab has been given?

5. How much money is given per person or jab to hospitals / GPs / NHS trusts etc?

6. Have all hospitals / GPs / NHS trusts been informed of the yellow card reporting scheme?

7. If so, how was this carried out and when?

8. Is there a financial incentive for hospitals / GPs / NHS trusts to report vaccine reactions to the yellow card scheme?

Deaths involving COVID-19 after receipt of COVID-19 vaccination

In table 1 of the recently published Deaths involving COVID-19 by vaccination status, England: deaths occurring between 2 January and 2 July 2021, we have provided deaths involving COVID-19 within 21 days of first and second vaccination, and deaths 21 days or more after first or second dose.

Unfortunately, we do not hold analysis on this data broken down by brand.

However, we have undertaken analysis of the ages of the 256 breakthrough deaths in tables 1 and 2 of the publication.

This information is not broken down further by age as this would constitute personally identifiable information.

This is the latest data we hold. However, we are planning a future publication as we obtain more data. The publication date will be announced on our Release Calendar.

COVID-19 deaths with positive PCR test within 28 days of vaccination

We do not hold analysis showing deaths involving COVID-19 with a positive PCR test taken 28 days after vaccination by brand or age bands. This information would need to be created. Under the Freedom of Information Act 2000, Public Authorities are not obligated to create information in order to respond to requests.

However, we do hold similar analysis showing "Breakthrough cases and deaths". "Breakthrough cases" are where infection has occurred in someone who is fully vaccinated, whereas we define a "breakthrough death" as a death involving COVID-19 that occurred in someone who had received both vaccine doses and had a first positive PCR test at least 14 days after the second vaccination dose.

In total, there were 256 breakthrough deaths between 2 January and 2 July 2021.
