Unfortunately, we do not hold any figures on people who have been killed or hospitalised as a result of drivers illegally using their mobile phone while driving. Your request may be best placed with the Department for Transport or the Department for Health. A search across government departments for the information you require has revealed the following of which you may find helpful.
The Department for Transport publish an annual report on Reported Road Casualties in Great Britain, and the latest report was published in September 2016 which covers the calendar year 2015. In the accompanying tables for the report, particularly table RAS50007, you can find Casualties in reported accidents by contributory factor and severity for the years 2011-2015. “Driver using mobile phone” falls under the “Impairment or distraction” category, and the severity categories are; killed, seriously injured and slightly injured. The tables cover calendar years, so you may wish to contact the Department for Transport for your specific time period at roadacc.stats@dft.gsi.gov.uk and they may be able to facilitate your request.
In terms of people hospitalised as a result of drivers using their mobile phone while driving, NHS Digital publish an annual report on Hospital Admitted Patient Care Activity. The latest report was published in November 2016 which covers the financial year 2015-16. In the External causes table you can find the number of admissions broken down by cause. However, the table only contains numbers for people injured in collision with a vehicle, but it does not give details beyond that.
